Our Commitment to Patient Safety

At St Peter's Hospice, we are dedicated to providing safe, effective care at all times. However, we recognise that incidents can occur in any healthcare setting. We encourage a culture of openness, where all incidents - big or small, including near misses - are reported to help us learn and continuously improve patient safety.

What is the Patient Safety Incident Response Framework (PSIRF)?

The PSIRF is the new approach adopted by healthcare organisations, including the NHS and hospices, to manage and learn from safety events. It replaces the Serious Incident Framework (SIF) and marks a shift toward more compassionate, reflective responses to incidents.

This framework helps us build a strong, effective patient safety system that focuses on:

  • Compassionate engagement with those affected by safety incidents
  • A thoughtful, system-based approach to learning
  • Balanced and proportionate responses to incidents
  • Supportive oversight aimed at continuous improvement

PSIRF Policy and Plan

The aim of our PSIRF policy and plan is to provide guidance and clarity as to how we intend to embed the new framework and respond compassionately to patient safety incidents.
