Hospice at Home Referral - St Peter's Hospice

Service Description

Hospice at Home provides care and support to patients with a high level of need in the last 2-4 weeks of life to enable them to stay in their home.

Care is provided by senior healthcare assistants with registered nurses providing support and assessment.

Care is delivered either once or twice a day between 08:00 and 20:00 with limited night cover available.

Care is planned on a needs-led basis, our visits are not timed so can adapt to individual need and changing situations.

Referral criteria

Patient has been assessed by a healthcare professional to be in the last 2-4 weeks of life and actively deteriorating
Preferred place of death is 'home'
Patient has consented to referral to Hospice at Home and the service has been explained to them.

Hospice at Home team

Prioritisation of care will be to patients with:

Rapid deterioration
Unstable or complex symptoms that need high intensity of care
Limited or no package of care
High level of family support needed

Referrals should be made on 0117 915 9508, 09:00 – 17:00, seven days a week.

Please call to discuss if you are unsure if your patient meets our criteria.

Please Note

Patients will be discharged if care is no longer needed and care may be transferred to a CHC FT package if appropriate to do so.
Referral for CHC FT, including Marie Curie if nights required, should be made by the referrer to ensure all avenues of support are accessed for the patient and their family.
