Hospice at Home provides care and support to patients with a high level of need in the last 2-4 weeks of life to enable them to stay in their home.
Care is provided by senior healthcare assistants with registered nurses providing support and assessment.
Care is delivered either once or twice a day between 08:00 and 20:00 with limited night cover available.
Care is planned on a needs-led basis, our visits are not timed so can adapt to individual need and changing situations.
Referral criteria
Patient has been assessed by a healthcare professional to be in the last 2-4 weeks of life and actively deteriorating
Preferred place of death is 'home'
Patient has consented to referral to Hospice at Home and the service has been explained to them.
